At the Bossier Chamber of Commerce, we recognize how important our military and first responders are to our community. The annual Patriot Awards Banquet honors Bossier Parish military personnel, law enforcement, fire/EMS, and Homeland Security. This event honors men and women in several categories based on criteria from a local selection committee as well as the Patriot of the Year.

Nominees are evaluated using the following criteria:

• Demonstrates excellence in job performance in extraordinary circumstances
• Provides valuable service by volunteering their time to improve the quality of life for others in the community
• Goes above and beyond their call to duty

Please submit 2 pictures and criteria for nomination on a separate page and attach them to this form. The pictures are for the ceremony & will be returned if needed. Additional articles, awards, pictures, etc. related to the supporting comments can be submitted as supporting information. Nominees will be contacted by the Bossier Chamber to submit a personal bio. Awards will be presented at the Patriots Award Luncheon on September 11, 2012 at the Bossier Civic Center, 11am to 1pm.
